It is widely recognised that the transition point at the age of eighteen is a challenging one for young adults but particularly if they have mental health problems.

Somerset Partnership is committed to the development of an improved new 0-25 pathway, with the aim of ensuring a smooth transition between services as needed.

The post-holder will play a key role in the further development of the Connect18 service.

Connect18 are looking to a recruit highly motivated individual to work with us to modernise and improve the transition for young people. The post holder will work across the 0-25 pathway, working primarily care coordinating the 17 plus age group to ensure that the pathways between children and adult services are as smooth as possible. It will also involve contributing to the management of young people with a range of complex mental health needs.

If you are a Registered Mental Health Nurse, Social Worker OR Occupational Therapist with Mental Health experience, who has an understanding of working with young people and families, we would love to hear from you.

The post will be based in the East of the county with the base being negotiated within this area.
